Job Description
We are an enthusiastic multi-disciplinary team with the primary aim of ensuring a safe and timely discharge of patients across a number of local authorities.
Based in Whiston hospital A&E department, and working within BASW COVID 19 guidance we complete assessments remotely whenever possible. The work is diverse, challenging and rewarding, requiring individuals to be able to work independently and contribute to the working of a larger multi-disciplinary team. The team is a mixed skill group with a range of experience who provide a supportive working environment.
We work on a shift rota basis between 8am and 10pm x 7 days. You will be expected to provide social care advice and information to A&E staff to support and facilitate discharges, complete social care assessments and MC capacity assessments, attend MDT/ DTL meetings (via teams). Identify and report safeguarding concerns as the arise, and support the wider MDT within contact cares out of hours.
You must have:
- A professional qualification in social work and experience of working with Adults.
- Social Work England registration.
- Post qualified experience.
- Enhanced DBS/Update service.
- References required for the last 5 years of employment.
