Job Description
Working in a busy Carers Network team, worker will be required to undertake Carers Assessments, Needs Assessments for the cared for (when required), participate in a Duty rota and support assistant care co-coordinators. Travelling and home visits may be required.

You must have:
- A professional qualification in social work and experience of working with Adults
- Social Work England registration.
- Post qualified experience.
- Enhanced DBS/Update service.
- References required for the last 5 years of employment.
