Job Description
experienced Social Workers to carry out assessments of patients ready for discharge from hospital. Need experience of completing assessments, MCA’s, making BI decisions, Continuing healthcare process and able to complete accurate CHC checklists and DST's with CCG nurses. Will work in a team made up of a manager 3 team leaders (advanced practitioners) 8 Social Workers (2 of which are agency and 2 ASYE) and 6 service coordinators working with Calderdale residents across 2 hospitals and in short term placements across 3 local authorities (Calderdale, Kirklees and Bradford) They will be working with multiple professionals to support the assessment process, i.e. therapy teams inc. specialist stroke, SALT, ward staff, discharge coordinators etc. Candidates will need to have received 2 covid vaccines to be allowed into carehomes. Calderdale council use the CIS system for recordings, experience with this system is desirable but support will be given during induction if candidates not familiar. Candidates will predominantly work in the hospital discharge section of the team but may be required to support review and reassessment section dependent on pressures.
You must have:
- A professional qualification in social work and experience of working with Adults.
- Social Work England registration.
- Post qualified experience.
- Enhanced DBS/Update service.
- References required for the last 5 years of employment.
