Job Description
Social worker to assess patients that are residents of Warrington, with Warrington Hospital and out of area hospitals. They need to have a working knowledge of the care Act 2014, capacity Act 2005 and Best interest legislation.
Experience of a hospital team would be desirable due to the constant challenges and MDT working that takes place.
We are a small team that needs to facilitate timely safe discharges.
We assess within the hospital and follow clients through on discharge to assess for the 4 week review. therefore a car is essential

You must have:
- A professional qualification in social work and experience of working with Adults
- Social Work England registration.
- Post qualified experience.
- Enhanced DBS/Update service.
- References required for the last 5 years of employment.
