Job Description
We have a post available in our REACT Team to start 15th December.
The team works closely with NHS colleagues to explore alternatives to care home and hospital admission.
Integrated working with NHS, crisis response. Social Care assessment and care planning,.

You must have:
- A professional qualification in social work and experience of working with Adults
- Social Work England registration.
- Post qualified experience.
- Enhanced DBS/Update service.
- References required for the last 5 years of employment.
