Job Description

Key Responsibilities The post holder will be responsible and accountable for:  Ensuring their individual and service performance targets are met.  Working with colleagues to ensure the team functions successfully in support of the Council’s corporate and service objectives.  To provide an assessment and care management service to older people, people with disabilities and their carers in the community, within the framework of relevant legislation, Council Policies and procedures.  Hold a caseload with appropriate supervision and advice.  To contribute to the development of new forms of work and service under the supervision of the Team Manager, Senior Social Worker or Care Manager / Social Worker.  On the basis of an assessment of need to devise and implement an appropriate care plan. This may involve arranging appropriate services in the public and private sector such as home care, day care or residential/nursing care, within available resources.  To participate in meetings as required.  To be prepared to participate in such other appropriate duties as may be allocated from time to time by your Team Manager, Senior Social Worker or Care Manager/Social Worker.  To take part in the office rotas when required e.g. duty, emergencies.  To support students on placement where considered appropriate.  To be responsible for promoting continuing personal and professional development of self through the staff development process.  To maintain and enter records of social work activities in accordance with approved Policies and Procedures, including the use of a computer system (i.e. Carefirst, ESCR)  To comply with safe systems of work in accordance with the Directorate’s Health, Safety and Welfare Policies and Procedures.  To develop an understanding and awareness of issues related to equal opportunities and anti-discriminatory practice and to demonstrate this in carrying out their duties.  To have responsibility for promoting continuing personal and professional / vocational development of self through development processes.  To develop specialist skills which may entail further relevant training.  The post holder may be required to undertake other duties, commensurate with the grade and responsibilities of the post in any geographical area. These are illustrative duties. The post holder will be expected to become involved in a range of work to enable the service to respond effectively to the changing requirements of the Council and changes affecting the workforce.

  • A recognised qualification (NVQ Level 2 or above in Health and Social Care) and experience of working in s

  • A valid enhanced DBS with Adult Barred List Checked.
